Outcome measures
Primary outcome
Fear of falling will be mapped with the Falls efficacy scale international
(FES-I). Physical activity will be measured by an accelerometer. The Fitbit
Classic will be used in this study. Seen the limitations of the Fitbit Classic
(it is not suitable in water), physical activity will also be measured through
the Lasa Physical Activity Questionnaire, in short LAPAQ. The physical capacity
will be measured through the Short Physical Performance Battery, in short SPPB.
Balance will be measured through the Timed up and go test, in short TUGT.
Muscle strength of the lower extremities will be measured through the Timed
Chair Stand Test, in short TCST. Endurance will be measured through the
2-minute step test, in short 2MST. Flexibility will be measured trough the
Chair Sit and Reach Test, in short SRT.

Background summary
In the Netherlands, during the past few years, there*s an increase in ageing
population. According a prognosis made by the Centraal Bureau van de Statistiek
(CBS) and the Rijksinstituut voor Volksgezondheid en Milieu (RIVM), the
population of elderly people, who are at least 65 years of age, will increase
till 4,8 million in 2040. Not only the number of elderly people will grow,
their age will grow to. Growing older comes with deficits, lots of seniors
increasingly experience physical and mental issues. A problem of frequent
occurrence with seniors who suffer from chronic illnesses is *fear of falling*.
Lots of studies show statistics of prevalence of *fear of falling*. Prevalence
of fear of falling upon seniors* variates from 20,8 percent to 85 percent, this
according to an international systematic review consisting of 21 studies. This
variation can be explained due diversity in the study populations and the
measuring instruments which were used to describe the degree of *fear of
falling*.
Study objective
The main purpose of this study is to answer the research question: What is the
relation between on the one hand fear of falling and physical activity and on
the other hand fear of falling and physical capacity in elderly people from 65
years of age and older before and after following a personalized treatment from
a physical therapist.
Study design
This study has an observational design, for 12 weeks.
Intervention
The intervention consists of usual care. The subjects undergo a personalized
program from a physical therapist that is based on the latest guidelines and
interventions from the Koninklijk Nederlands Genootshap voor Fysiotherapie
(KNGF).
Study burden and risks
Risks of participating this study are minimal, because participants undergo
*usual care*. The physical performance tests have no additional risks because
they all have been standardized and are fully reliable. This is based on
evidence based practice and is developed in consultation of a multitude of
health care professionals.

- Referral from the general practitioner due to chronic condition
- FAC 3 or more
Exclusion criteria
If an older adult is insufficiently instructible after assessment by the physical therapist, he or she can not participate in this study.
